How it works

Mechanism of action

D3 converted to 25(OH)D then 1,25(OH)2D (calcitriol), activating VDR receptors for calcium/phosphorus absorption and immune modulation. K2 activates osteocalcin (directs Ca to bone) and matrix Gla protein (prevents vascular calcification).

Vitamin D and K Supplementation for Bone and Cardiovascular Health

Nutrients (2017)

Combined D3+K2 improved bone mineral density and reduced arterial stiffness more than either alone.

Safety profile

Monitoring & safety information.

Commonly reported side effects

  • Generally very well tolerated
  • Hypercalcemia (rare, at very high D3 doses)
  • GI upset

Less common but serious risks

  • Vitamin D toxicity with excessive dosing (>10,000 IU/day chronic)
  • Hypercalcemia

Generally not appropriate for

  • Hypercalcemia
  • Granulomatous disease (sarcoidosis)
  • Concurrent warfarin (K2 antagonizes warfarin)

Typical monitoring

  • 25(OH)D level (target 40 to 60 ng/mL)
  • Serum calcium
  • PTH
  • INR if on warfarin
