Frequently asked questions.
Everything you need to know about Marek's Guided Optimization® Program, from pricing and labs to treatments and coaching.
Getting started
Starting your protocol.
Your $299 intake assessment includes a health history review and a 45-minute consultation with your dedicated health coach. During that session, you and your coach build a personalized lab panel based on your goals, starting with your foundational panel and adding any additional markers your coach recommends. Labs are purchased separately, starting at $450 for your foundational panel. After your labs are drawn, your coach reviews every result with you, and a partnered licensed medical provider builds your treatment protocol. Ongoing treatment averages $80–200/month with no subscription required.
Your regular doctor has 7 minutes. We have a dedicated multi-disciplinary team of experts thinking about your health year round. An expert Health Coach and partnered licensed medical provider in your back pocket. A team who knows your full health history, reviews every lab result with you screen by screen, and checks in with you, month in, month out, holding you accountable to your goals. We are incentivized to see you succeed and that is why we get results. Our tests go deeper than anything you have done before. We test 1,000+ available biomarkers vs. the 10–15 typically ordered at an annual physical, and we use the most accurate testing methods available, including LC/MS for testosterone.
Lab results are typically available within 14 days of your blood draw. From there, your coach reviews them with you and your provider builds a protocol, total time from labs to protocol is about 2–3 weeks.
Yes, Marek's Guided Optimization® Program is available in all 50 states. Specific treatments and diagnostics may be limited in certain regions due to state regulations.
Marek currently only serves U.S.-based residents. American citizens working overseas with APO boxes may qualify for select item shipments, inquire with your Health Coach.
The program
How the protocol works.
Physical Examination: An annual physical exam (every 12 months) from a qualified MD, DO, PA, or NP is required; documentation is needed before starting and continuing treatment. Medical Provider Consultations: Initial telehealth consultation with your medical provider, follow-up visit within 6–12 weeks post-treatment initiation, routine visits every 6 months, and additional consultations as recommended. Diagnostic Lab Work: Initial complete lab tests before your first provider consultation, follow-up basic labs 6–12 weeks after starting treatment, ongoing basic labs every 6 months, and additional diagnostics as recommended.
We match you with a health coach based on your specific goals, health history, and preferences. If at any point you'd like to switch coaches, just let us know, we'll reassign you, no questions asked.
After your first year, you move into flexible maintenance. There's no mandatory renewal or annual fee. You continue with follow-up labs and treatment refills at your own pace.
We recommend follow-up labs every 3–6 months to track your progress, adjust dosing, and ensure safety. Your coach will help you determine the right cadence based on your protocol.
That almost never means "there's nothing we can do." It usually means the real issue isn't what you thought it was, and that's exactly what our process is designed to uncover. Many clients come in thinking they need TRT, only to discover the root cause is actually a thyroid issue, a prolactinoma, or a lifestyle factor that no medication can fix. That's not a dead end, that's a better answer. Our providers have access to over 700 medications and hundreds of supplements, and our Health Coaches are experts in the lifestyle fundamentals that support everything: nutrition, exercise, sleep, and circadian rhythm. Whether your protocol ends up being a prescription, a supplement stack, a complete lifestyle overhaul, or some combination, you'll leave with a clear path forward.
You can obtain a physical from any qualified MD, DO, PA, or NP, including at urgent care clinics or similar facilities. Marek provides a physical exam form available on the website that you can bring to your appointment.
Physicals completed within 3 months of your application are acceptable. After that, 12-month intervals are required.
Labs & testing
Your lab work, explained.
Your foundational panel covers hormones, thyroid function, metabolic health, cardiovascular risk, organ function, and key nutrients, coach-selected from 1,000+ available biomarkers. This is broader than the 10–15 markers typically ordered at an annual physical.
We partner with Quest Diagnostics and Labcorp, giving you access to over 4,000 patient service center locations nationwide. You'll receive a lab requisition and can walk into any location, no appointment needed at most sites.
Yes. We use liquid chromatography–mass spectrometry (LC/MS) for testosterone testing, the gold standard assay method. Most primary care offices use cheaper immunoassay methods that can be inaccurate by 20–30%. LC/MS gives you precise, reliable results.
Typical turnaround is 5–7 days for basic panels and 10–14 days for larger panels, depending on testing complexity and methodology.
Marek requires customized, full lab panels including high-sensitivity and the most up-to-date testing methods, beyond what standard check-up panels cover. A minimum full panel is required to begin treatment, with testing required every 6 months.
Treatments
Medications and therapies.
We prescribe from a formulary of 700+ therapies, including testosterone replacement therapy (TRT), GLP-1 receptor agonists (semaglutide, tirzepatide), peptides, thyroid support, and more. Your provider selects treatments based on your labs, symptoms, and goals.
Qualification extends beyond simple hormone level markers. We evaluate symptoms, lab work, health history, and overall health together. Individuals with concerning markers may show no symptoms, while others with normal markers experience significant symptoms. A provider consultation is required to determine eligibility through lab assessment, medical history review, and symptom evaluation.
Our formulary includes 700+ therapies. Contact your health coach to discuss specific medications and available products for your situation.
State and federal regulatory requirements vary significantly. Some states restrict days supply, medication imports, or ban certain substances entirely. Marek complies with all applicable laws based on your state of residence.
The process involves multiple steps: staff review and pharmacy submission, physician review and approval, compounding pharmacy preparation (up to 48 hours), pharmacist review, shipping department inspection, and shipping. Marek uses only FDA-approved 503A/503B compounding pharmacies, enabling customized delivery routes and medication strengths.
Pricing & insurance
Cost and coverage.
We operate outside of the traditional sick-care ecosystem so we don't bill health insurance directly. However, most clients can use HSA or FSA funds to cover their costs. We can provide invoices and lab requisitions for your records. Many clients find the out-of-pocket cost comparable to specialist co-pays, with significantly more depth, personalization, and ongoing support.
Your ongoing monthly cost covers your prescribed treatment, continued health coaching, follow-up lab panels, portal access, and provider oversight. Everything you need to stay on protocol, no hidden fees.
No. There's no subscription, no minimum commitment, and no cancellation fee. You pay for each service as you go, intake, labs, treatment, follow-ups. If you decide to pause or stop, there's nothing to cancel.
Yes. The majority of Marek Health services, including lab panels, provider consultations, and prescription treatments, are HSA/FSA eligible. We provide itemized invoices that you can submit for reimbursement.
As a direct-pay elective service, we do not provide diagnostic codes (ICD-10) or procedure codes (CPT), as we do not seek insurance reimbursement.
As an elective cash-pay service, we cannot generate Letters of Medical Necessity or complete forms for insurance, disability, military, employment, or educational purposes. Contact your primary healthcare provider for this documentation. We do provide invoices and lab requisitions upon request.
