Testosterone EnanthateTestosterone Enanthate

Testosterone enanthate is a long-acting esterified testosterone with a half-life of approximately 4 to 5 days, slightly shorter than cypionate. Widely used globally for male hypogonadism and TRT. At Marek, protocols favor daily or three-times-weekly subcutaneous microdosing for stable levels, optimal, not convenient.

How it works

Mechanism of action

The enanthate ester is cleaved by tissue esterases, releasing free testosterone that binds androgen receptors. Undergoes aromatization to estradiol and 5-alpha reduction to DHT. Enanthate's half-life technically permits weekly injections, but Marek protocols default to daily or three-times-weekly (TIW) subcutaneous microdoses to minimize peak-to-trough swings in testosterone and estradiol. Dose, frequency, and route are individualized using your labs and, where useful, Androgen Sensitivity Genetic Testing (AR CAG repeat length), so your provider can calibrate the protocol to your receptor biology, not just your serum blood level.

Every Marek client receives this baseline panel before a provider review. Your provider may add additional biomarkers based on your health history, symptoms, and goals, pulling from 1,000+ available markers. See more biomarkers →

Pharmacokinetics of Testosterone Enanthate

European Journal of Drug Metabolism and Pharmacokinetics (2019)

Weekly subcutaneous dosing achieves more stable serum levels with smaller peak-to-trough variation compared to biweekly IM dosing.
