Omega 3 Ethyl EstersOmega 3 Ethyl Esters

Pharmaceutical-grade omega-3 ethyl esters (the generic equivalent of Lovaza) deliver concentrated EPA and DHA at therapeutic doses well above what's available in OTC fish oil. Used primarily to lower elevated triglycerides and as an adjunct in cardiometabolic risk reduction. Ultra-purified to pharmaceutical standards, with no detectable mercury.

Significant triglyceride reduction (typically 20 to 45%)

Adjunct in cardiovascular risk-reduction protocols

Pharmaceutical grade, ultra-purified EPA + DHA, no detectable mercury

FDA-approved for severe hypertriglyceridemia (≥500 mg/dL)

How it works

Mechanism of action

Omega-3 ethyl esters concentrate eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) at therapeutic doses. They reduce hepatic synthesis of very-low-density lipoprotein (VLDL) triglycerides and increase fatty-acid β-oxidation, which lowers circulating triglyceride levels.

Beyond lipid effects, EPA/DHA influence membrane fluidity, eicosanoid signaling, and platelet aggregation, mechanisms hypothesized to contribute to the cardiovascular outcome differences seen in trials of high-purity formulations.

EPA and DHA are also the direct precursors to specialized pro-resolving mediators (SPMs), the resolvins, protectins and maresins, which act locally at sites of inflammation or tissue damage to actively switch off the inflammatory response and restore normal cellular function. This makes omega-3s one of the few compounds that help resolve inflammation rather than merely suppress it.

EPA also incorporates directly into atherosclerotic plaque, particularly vulnerable thin-cap plaque, where it improves plaque stability and reduces plaque inflammation and foam-cell formation. In the EVAPORATE trial, high-dose EPA ethyl ester produced regression of low-attenuation (vulnerable) plaque on serial CT angiography versus continued progression on placebo. And the stakes are high: population analyses rank low omega-3 intake among the leading dietary risk factors for preventable death in the United States, the same landmark analysis that put smoking, high blood pressure, and obesity at the top of the overall list.

The research behind Omega 3 Ethyl Esters.

REDUCE-IT: Icosapent ethyl in patients with elevated triglycerides on statin

New England Journal of Medicine (2019)

8,179 statin-treated patients with elevated triglycerides. The high-dose EPA-only formulation was associated with a 25% relative reduction in major adverse cardiovascular events versus placebo.

Commonly reported side effects

  • Fishy aftertaste or eructation
  • Mild GI upset (typically dose-related)
  • Nausea (uncommon)

Less common but serious risks

  • Atrial fibrillation, modestly elevated incidence in some high-dose trials
  • Bleeding risk, clinically significant only at very high doses or with anticoagulant co-use
  • Allergic reaction in patients with fish or shellfish hypersensitivity

Generally not appropriate for

  • Known hypersensitivity to fish or shellfish
  • Active bleeding disorder
  • Use caution if on anticoagulants, discuss with provider

Typical monitoring

  • Triglycerides and lipid panel
  • ApoB and non-HDL cholesterol
  • hsCRP (if used for inflammatory adjunct)
  • Symptom review for arrhythmia or bleeding

FAQ

Quick answers.

Eligibility, dose, and duration are determined by a partnered licensed medical provider based on your complete labs and medical history. A prescription is not guaranteed, your provider may recommend Omega 3 Ethyl Esters, a different therapy, or a non-pharmacologic approach.
OTC fish oil is a dietary supplement with variable potency and purity. Omega 3 Ethyl Esters are pharmaceutical-grade, concentrated to a known EPA/DHA ratio, and dosed therapeutically (typically 2 to 4 g/day), the doses required for clinically meaningful triglyceride reduction.
Patients with elevated or severely elevated triglycerides (≥500 mg/dL FDA-approved indication; clinical judgment in the 150 to 500 mg/dL range), patients on statin therapy with residual cardiometabolic risk, or those with specific inflammatory protocols. Your provider individualizes based on labs.
Most commonly: fishy aftertaste / eructation, mild GI upset. Less common but worth knowing: a small increase in atrial fibrillation incidence seen in some high-dose trials, and a theoretical bleeding-risk increase that's only clinically significant at very high doses or with anticoagulants.
