NAD+ (Nicotinamide Adenine Dinucleotide)NAD+ (Nicotinamide Adenine Dinucleotide)

Essential coenzyme for cellular energy production, DNA repair, and sirtuin activation. NAD+ levels decline ~50% between ages 40 to 60. Supplementation aims to restore cellular NAD+ for mitochondrial function, DNA repair, and anti-aging sirtuin activity.

How it works

Mechanism of action

NAD+ serves as electron carrier in mitochondrial oxidative phosphorylation (complexes I and II), substrate for sirtuins (SIRT1-7) which regulate metabolism and gene silencing, substrate for PARPs involved in DNA repair, and substrate for CD38-mediated calcium signaling.

Commonly reported side effects

  • IV infusion: chest tightness, flushing, nausea (rate-dependent)
  • Nasal: nasal irritation
  • Headache
  • GI upset

Less common but serious risks

  • IV infusion reactions if administered too rapidly
  • Theoretical: cancer cell NAD+ utilization

Generally not appropriate for

  • Active malignancy (relative, NAD+ fuels rapidly dividing cells)
  • Pregnancy

Typical monitoring

  • NAD+ metabolome (blood spot test)
  • Energy and cognitive symptoms
  • Liver function
