FenofibrateFenofibrate

PPAR-alpha agonist for hypertriglyceridemia and mixed dyslipidemia. Reduces microvascular complications in T2DM (retinopathy 37% reduction in FIELD trial). Also has uricosuric effects. Preferred fibrate with statins (lower myopathy risk vs gemfibrozil).

TG reduction 30 to 50%

HDL-C increase 10 to 20%

Diabetic retinopathy reduction 37% (FIELD)

How it works

Mechanism of action

Activates PPAR-alpha in hepatocytes and muscle, upregulating lipoprotein lipase and fatty acid oxidation genes. Increases triglyceride clearance, reduces VLDL production, raises HDL-C, and shifts LDL to large-buoyant particles.
