1. Sponsor
The Marek Health Win Your Year Sweepstakes (the "Sweepstakes") is sponsored by Marek Health LLC, 35 W. Huron Street, Suite 1000, Pontiac, Michigan 48342 ("Sponsor").
2. Promotion Period
The Sweepstakes begins at 12:00:00 a.m. Eastern Time ("ET") on October 28, 2026 and ends at 11:59:59 p.m. ET on November 8, 2026 (the "Promotion Period"). Sponsor’s computer system is the official timekeeping device for the Sweepstakes.
3. Eligibility
The Sweepstakes is open only to legal residents of one of the fifty (50) United States or the District of Columbia, excluding residents of New York, New Jersey, and Rhode Island, who are at least twenty-one (21) years of age at the time of entry.
An entrant must not have been an active Marek Health client at any time on or before October 27, 2026. An otherwise eligible entrant who becomes a Marek Health client on or after October 28, 2026 remains eligible to enter and win. Sponsor’s records will be used to verify whether an entrant was an active Marek Health client on or before October 27, 2026.
Employees, officers, directors, contractors, and agents of Sponsor, persons directly involved in administering or promoting the Sweepstakes, and the immediate family members and household members of each such person are not eligible. For purposes of these Official Rules, “immediate family” means spouse, parent, child, sibling, and the spouse of each such person, whether or not residing in the same household.
The Sweepstakes is subject to all applicable federal, state, and local laws and regulations.
4. How to Enter; No Purchase Necessary
During the Promotion Period, an eligible individual may enter by completing the Sweepstakes entry process, which may include a brief pre-entry questionnaire followed by the designated online Sweepstakes sign-up form with the information requested by Sponsor. No purchase, payment, Guided Optimization® enrollment, use of Marek Health services, marketing opt-in, or particular questionnaire response is required to enter or win.
The pre-entry questionnaire may ask about an entrant’s personal priorities, preferred forms of support, common obstacles when pursuing goals, prior investment in coaching or personal-development programs, and readiness to invest in themselves. Completion of the questionnaire may be required to proceed to the entry form, but questionnaire responses are not eligibility criteria, will not affect an entrant’s odds of winning, and will not be used to exclude an otherwise eligible entrant from the random drawing. Sponsor may use questionnaire responses for campaign analytics and, where an entrant separately and affirmatively opts in to marketing, for segmentation and personalization consistent with the scope of that consent, Sponsor’s Privacy Policy, and applicable law.
After submitting the Sweepstakes sign-up form, the entrant must confirm the entry by clicking the unique confirmation link sent to the email address provided. Submission of the form alone does not constitute a completed entry. The confirmation link must be clicked no later than 11:59:59 p.m. ET on November 8, 2026. Only timely confirmed entries will be eligible for and included in the random drawing. Sponsor may send transactional confirmation and reminder emails for the limited purpose of completing or administering the Sweepstakes entry. Confirming an entry does not constitute consent to receive general marketing communications from Sponsor.
Limit one (1) entry per person during the Promotion Period, regardless of the number of email addresses, telephone numbers, accounts, devices, or other identifiers used. Entries generated by script, macro, bot, automated means, or other methods intended to circumvent the entry limit are prohibited.
Entry into the Sweepstakes does not constitute consent to receive general marketing communications from Sponsor. Sponsor may present a separate, optional, unchecked marketing-consent checkbox with the entry form. An entrant who affirmatively opts in may be added to Sponsor’s general marketing lead pool, including after the Sweepstakes, and may receive marketing communications only to the extent consistent with the scope of that consent and applicable law. Declining, or later withdrawing, marketing consent will not affect the entrant’s ability to enter, eligibility, odds of winning, or entitlement to a prize. Regardless of marketing consent, Sponsor may use entry information as reasonably necessary to administer the Sweepstakes, verify eligibility, contact potential winners, award prizes, comply with law, and maintain required records.
Incomplete, late, corrupted, or otherwise noncompliant entries may be disqualified in accordance with these Official Rules. In the event of a dispute regarding the identity of an entrant, Sponsor may treat the authorized account holder of the email address submitted with the entry as the entrant, subject to verification of eligibility.
5. Random Drawing; Odds
On or about November 9, 2026, Sponsor or its designee will select two (2) potential winners by random drawing from among all eligible entries received during the Promotion Period. The odds of winning depend on the number of eligible entries received. Selection as a potential winner does not constitute final confirmation of eligibility or entitlement to a prize.
6. Prizes
Two (2) prizes are available. Each confirmed winner will receive one (1) Marek Health promotional credit with an approximate retail value (“ARV”) of $3,500, for a total ARV of $7,000.
The credit may be applied to eligible Guided Optimization® enrollment, diagnostic testing, provider examinations, medications, and supplements available through or in connection with Marek Health, subject to these Official Rules, applicable law, availability, and applicable clinical requirements.
The ARV of each prize is fixed at $3,500 regardless of whether a winner purchased or enrolled in Guided Optimization® or another Marek Health service before being confirmed as a winner. Any credit applied with respect to a prior Guided Optimization® enrollment payment will be included within, and not added to, the $3,500 prize value. In no event will a prior purchase increase the value of a winner’s prize or the winner’s odds of winning.
Prescription medications, laboratory testing, diagnostic testing, provider services, and other clinical services remain subject to applicable law, provider licensure, medical necessity, and the independent clinical judgment of the applicable treating provider. Winning the Sweepstakes does not guarantee that any particular medication, laboratory test, diagnosis, treatment, or other clinical service will be ordered, prescribed, dispensed, furnished, or determined to be clinically appropriate.
The prize credit expires one (1) year after Sponsor confirms the entrant as an official winner. Any unused balance at expiration is forfeited and has no cash value. The prize is nontransferable, may not be assigned, and may not be redeemed or substituted for cash. Sponsor may substitute a prize of equal or greater value if necessary and permitted by law.
Laboratory testing and other services furnished through the prize will not be submitted to insurance or another third-party payer. A winner is responsible for any charges that exceed the available prize credit.
All prize details not expressly specified in these Official Rules will be determined by Sponsor, subject to applicable law and the independent clinical judgment of any treating provider.
7. Potential Winner Notification and Verification
Potential winners will be contacted using the email address and/or telephone number provided at entry. Sponsor may require reasonable proof of age, residency, identity, entry compliance, and other eligibility criteria.
A potential winner must respond and return all required winner documentation within five (5) calendar days after Sponsor’s first notification attempt. If a potential winner fails to timely respond or return the required documentation, is determined to be ineligible, cannot be contacted, declines the prize, or otherwise fails to comply with these Official Rules, the potential winner may be disqualified and the prize may be forfeited. Sponsor may then select an alternate potential winner by random drawing from the remaining eligible entries and may repeat that process as reasonably necessary to award the advertised prizes, subject to applicable law.
A potential winner is not an official winner until Sponsor verifies eligibility and compliance and confirms the winner in writing or by other documented communication.
8. Required Winner Affidavit, Prize Acceptance and Non-Clinical Publicity Release
As a condition of accepting a prize, each potential winner must timely execute and return Sponsor’s Winner Affidavit of Eligibility, Prize Acceptance and Publicity Release, except to the extent a particular provision is prohibited by applicable law.
The Winner Affidavit permits Sponsor, where permitted by law, to use the winner’s name, city and state of residence, image, likeness, voice, photograph, video, fact of entry or winning, nature and value of the prize, and non-clinical statements concerning the Sweepstakes and receipt of the prize for advertising, publicity, promotional, and related commercial purposes without additional compensation or further approval.
The Winner Affidavit also requires reasonable cooperation, at a mutually agreeable time and without out-of-pocket cost to the winner, in one brief photograph, video recording, or interview concerning the Sweepstakes, the winner’s selection, and receipt of the prize. A winner is not required to make any positive or favorable statement about Sponsor or to discuss health information, medical history, diagnosis, treatment, laboratory results, medications, or health outcomes.
The Winner Affidavit does not authorize the use or disclosure of protected health information or clinical treatment information for marketing.
10. Public Announcement and Endorsement Disclosure
Sponsor may publicly announce a winner only after eligibility has been verified and the required Winner Affidavit has been signed. Sponsor will not announce a potential winner as a final winner before verification is complete.
If Sponsor uses a winner’s statement or experience as an endorsement or testimonial, Sponsor will disclose, as appropriate, that the individual was a Sweepstakes winner and received a $3,500 Marek Health credit. Any testimonial or endorsement must reflect the winner’s honest opinions and actual experience.
11. Taxes
Each winner is solely responsible for all federal, state, and local taxes, if any, arising from acceptance or use of the prize. Sponsor may require a completed IRS Form W-9 and other reasonably necessary tax documentation and will issue any tax reporting required by applicable law.
12. Disqualification and General Conditions
Sponsor may disqualify an entrant who, based on the facts reasonably available to Sponsor: (a) submits more than one entry or uses multiple identities, email addresses, telephone numbers, accounts, automated methods, or other means to obtain additional entries; (b) provides false, incomplete, or misleading eligibility information; (c) tampers with or attempts to interfere with the entry, administration, random drawing, or winner-verification process; (d) violates these Official Rules; or (e) engages in fraud, deception, harassment, or other conduct that threatens the integrity or lawful administration of the Sweepstakes.
Sponsor’s decisions regarding administration and interpretation of these Official Rules are final to the extent permitted by law, provided that entries will not be arbitrarily removed, disqualified, disallowed, or rejected.
If fraud, technical failure, unauthorized intervention, force majeure, or another event beyond Sponsor’s reasonable control materially impairs the integrity or proper operation of the Sweepstakes, Sponsor may, subject to applicable law, suspend or terminate affected aspects of the Sweepstakes and select winners from eligible, non-suspect entries received before the impairment or take other lawful corrective action. Nothing in this provision authorizes Sponsor to change filed rules where applicable law prohibits such changes.
13. Release and Limitation of Liability
To the fullest extent permitted by law, by entering, each entrant agrees to release and hold harmless Sponsor and its affiliates, advertising and promotion agencies, vendors involved in administration of the Sweepstakes, and their respective officers, directors, employees, agents, successors, and assigns from claims arising out of the entrant’s participation in the Sweepstakes, the administration or random drawing, winner notification and verification, or the award, acceptance, possession, or non-clinical use of a prize.
The foregoing release does not apply to claims arising from the actual provision of professional health care services, gross negligence, willful misconduct, product claims that cannot lawfully be waived, or any other claim or right that applicable law does not permit to be released or limited.
To the extent permitted by law, Sponsor is not responsible for lost, late, incomplete, corrupted, misdirected, or undeliverable entries or communications caused by technical, network, platform, device, or transmission failures outside Sponsor’s reasonable control.
14. Privacy
Personal information collected in connection with the Sweepstakes, including information submitted through the pre-entry questionnaire and entry form, may be used to administer the Sweepstakes, verify eligibility, contact potential winners, award prizes, comply with legal and regulatory requirements, maintain required records, conduct campaign analytics, and as otherwise described in Sponsor’s applicable Privacy Policy. Questionnaire responses do not by themselves enroll an entrant in Sponsor’s general marketing programs. If an entrant separately and affirmatively opts in to marketing through the entry form or another valid consent mechanism, Sponsor may add that entrant to its general marketing lead pool and use the entrant’s contact information and questionnaire responses for marketing, segmentation, and personalization consistent with the scope of the consent, applicable law, and Sponsor’s Privacy Policy. Declining or withdrawing marketing consent does not affect Sweepstakes participation or eligibility. Sponsor will make its applicable Privacy Policy and any required notice at collection available at or before the point at which personal information is collected.
Health or clinical information obtained in connection with care or treatment is not authorized for marketing use by these Official Rules. Any marketing use or disclosure requiring a separate health-information authorization will be governed by the separate voluntary authorization described above.
15. Disputes; Governing Law
Except where prohibited by law, any dispute arising solely out of the Sweepstakes or these Official Rules will be governed by the laws of the State of Michigan, without regard to conflict-of-law principles.
To the extent permitted by law, any such dispute will be resolved individually and not as part of a class or representative action, and any judicial proceeding will be brought in a state or federal court located in Oakland County, Michigan. Nothing in this section waives any nonwaivable right or remedy available under applicable law.
To the extent permitted by law, recovery in any claim arising solely from the Sweepstakes will be limited to actual out-of-pocket costs associated with entering the Sweepstakes, and entrants waive claims for punitive, incidental, special, or consequential damages and attorneys’ fees, except where such limitations are prohibited by law.
17. Winner List and Official Rules
A list of confirmed winners will be available without charge after the winners have been finally determined. To request the winner list, email compliance@marekhealth.com with the subject line “Win Your Year Sweepstakes Winner List.” Requests should be received by January 31, 2027.
The full Official Rules should remain available through the Sweepstakes landing page or other designated location for the duration required by applicable law.

16. Social-Media Platforms
The Sweepstakes is not sponsored, endorsed, administered by, or associated with any social-media platform on which it may be advertised or discussed. Entrants release such platforms from claims arising solely from the administration of the Sweepstakes to the extent required by the applicable platform’s promotion rules and permitted by law.
